目的 为今后临床上更安全有效而广泛地重建颅内静脉窦进行初步尝试。方法 全麻下开颅切除8 只犬上矢状窦后, 以硅胶管为替代材料, 采用置管法和吻合法重建上矢状窦。通过数字减影矢状窦造影和经颅多普勒(TCD) 分别检测术前并比较术后1、2、4 和8 周重建上矢状窦的通畅性, 同时观察了组织学和超微结构。结果 矢状窦造影和TCD 证实5 只通畅,1 只不全阻塞,1 只仅近端堵塞, 另1 只术后丢失。术后除1 只呕吐外均无任何神经系统症状。组织学及超微结构表明术后8 周硅胶管和上矢状窦吻合处已有内皮细胞增生、爬行, 表面光滑未见血栓形成, 重建矢状窦通畅。结论 硅胶管作为重建颅内静脉窦的人工材料是可行的, 有很好的生物相容性, 无血栓源性, 再通性强, 无任何不良反应及毒副作用, 至少近斯效果良好, 待临床Ⅰ期实验。
Objective To make a preliminary attempt to rebuild the intracranial venous sinus more safely and effectively in the future. Methods After craniotomy of 8 dogs under general anesthesia, silicone tube was used as an alternative material, and the superior sagittal sinus was reconstructed by catheterization and anastomosis. The patency of the superior sagittal sinus was evaluated by digital subtraction sagittal sinusography and transcranial Doppler (TCD) before surgery and at 1, 2, 4 and 8 weeks postoperatively respectively. Histology and ultrastructure were also observed structure. Results Sagittal sinusography and TCD confirmed 5 patency, 1 incomplete obstruction, 1 proximal obstruction only and 1 postoperative loss. There was no neurological symptoms except one vomiting after operation. Histology and ultrastructure showed that there was endothelial cell hyperplasia and creeping in the anastomosis of silicone tube and superior sagittal sinus 8 weeks postoperatively, smooth surface without thrombosis and reconstruction of sagittal sinus patency. Conclusion Silicone tube is feasible as an artificial material for reconstructing intracranial sinus. It has good biocompatibility, no thrombus, strong recanalization, no adverse reactions and side effects. Clinical phase Ⅰ experiment.
