对比性研究了普鲁卡因复合麻醉下胆道手术病人前列腺素E1(PGE1)、硝普钠(SNP)降压30%前、中、后中心静脉压(CVP)及血浆醛固酮(Ald)、心钠素(ANP)浓度变化,并与未降压病人(对照组)进行了比较。结果发现PGE1、SNP组降压中CVP无显著改变而停用降压药后30min时均反跳性升高,对照组病人CVP全程无变化;降压中PGE1、ANP两组病人Ald显著升高,ANP显著下降,停药后30min时均回复基础水平。PGE1组病人降压中Ald、ANP变化幅度仅略小于SNP组(P>0.05)。提示PGE1控制性降压对Ald、ANP等水、钠调节激素变化的影响与SNP基本相当,并不明显抑制其应激性变化。降压期间CVP不变ANP显著下降说明ANP分泌与心房压力变化相关性差。
A comparative study was conducted to compare the effects of procaine E1 (PGE1), nitroprusside (SNP) 30% before, after, and after CVP and Aldosterone (Ald) Changes in the concentration of natriuretic peptide (ANP) were compared with those in non-depressurized patients (control group). The results showed that there was no significant change of CVP in PGE1 and SNP groups but no rebound in 30 min after antihypertensive treatment, while there was no change in CVP in the control group. The levels of Ald in PGE1 and ANP groups were significantly increased , ANP decreased significantly, 30min after stopping when all returned to the basic level. The levels of Ald and ANP in depressurization of patients in PGE1 group were only slightly less than those in SNP group (P> 0.05). Tip PGE1 controlled hypotension on Ald, ANP other water and sodium regulation of hormones and SNP basically the same, and did not significantly inhibit the stress changes. A significant decrease in CVP-invariant ANP during depressurization suggests a poor correlation between ANP secretion and changes in atrial pressure.
