目的 观察可溶性CD40配体(SCD40L)与冠心病合并重度牙周炎患者疾病发生、发展的作用及其相关性。方法 选取受试对象63例分为健康对照组(n=14)、重度牙周炎组(n=16)、冠心病组(n=19)、冠心病合并重度牙周炎组(n=14)。留取静脉血测定血清中SCD40L的浓度,观察各组中的表达水平。比较冠心病组及冠心病合并重度牙周炎组两组牙周参数(探诊深度(PD)、附着丧失(CAL)、探诊后出血(BOP)、菌斑指数(PLI)及余留牙数目)的不同。对SCD40L与探诊深度(PD)、附着丧失(CAL)、探诊后出血(BOP)、菌斑指数(PLI)进行相关性分析,观察SCD40L与各牙周参数是否存在相关性。 结果 与健康对照组比较,冠心病合并重度牙周炎组SCD40L明显升高(P<0.01);与重度牙周炎组、冠心病组比较,冠心病合并重度牙周炎组SCD40L水平升高(P<0.05);与冠心病组比较,冠心病合并重度牙周炎组余留牙数目多(P<0.05);与冠心病组比较,冠心病合并重度牙周炎组PD水平较深(P<0.05);与冠心病组比较,冠心病合并重度牙周炎组CAL及BOP增高(P<0.01)。SCD40L与PD存在明显正相关(P<0.01),SCD40L与BOP具有正相关性(P<0.05)。结论 SCD40L可作为预测冠心病合并重度牙周炎的炎性预测因子。
Objective To investigate the role and relationship of soluble CD40 ligand (SCD40L) and pathogenesis and development of patients with coronary heart disease and severe periodontitis. Methods Totally 63 subjects were divided into healthy control group (n = 14), severe periodontitis group (n = 16), coronary heart disease group (n = 19), coronary heart disease complicated with severe periodontitis group (n = 14) ). Venous blood was taken for determination of the concentration of SCD40L in serum and the expression level in each group was observed. Periodontal parameters (probing depth (PD), attachment loss (CAL), bleeding after probing (BOP), plaque index (PLI) and residual teeth number) were compared between CHD group and CHD group with severe periodontitis group. s difference. The correlation between SCD40L and probing depth (PD), attachment loss (CAL), bleeding after probing (POP) and plaque index (PLI) were analyzed to determine whether there was correlation between SCD40L and various periodontal parameters. Results Compared with healthy control group, SCD40L in patients with coronary heart disease and severe periodontitis was significantly increased (P <0.01). Compared with severe periodontitis group and coronary heart disease group, the SCD40L level in patients with coronary heart disease complicated with severe periodontitis was increased P <0.05). Compared with CHD group, the number of remaining teeth in CHD group was significantly higher than that in CHD group (P <0.05). Compared with CHD group, the PD level in CHD group was deeper (P <0.05 ). Compared with coronary heart disease group, CAL and BOP increased in patients with coronary heart disease complicated with severe periodontitis (P <0.01). There was a significant positive correlation between SCD40L and PD (P & lt; 0.01). There was a positive correlation between SCD40L and BOP (P & lt; 0.05). Conclusion SCD40L can be used as an inflammatory predictor of coronary heart disease complicated with severe periodontitis.
