目的研究CT灌注扫描定量对周围型肺癌放化疗后疗效判定的指导价值。方法根据RECIST疗效标准将我院56例周围型肺癌患者分为缓解组(31例)与未缓解组(25例),采取CT平扫及灌注成像在放化疗前后对两组患者进行诊断,先行常规肺部CT平扫定位病灶,6ml/s经肘注入碘海醇50ml,以3个时间序列进行扫描,后将图像上传至Leonardo工作站内进行分析得出肿瘤各项灌注参数(BV、BF、MTT、PS),比较缓解组与未缓解组之间CT灌注参数的差异。结果治疗前两组CT灌注参数值比较无统计学意义(P>0.05),治疗后缓解组BF、BV分别为(75.25±24.16)ml/min/100g、(4.16±1.24)ml/100g较本组治疗前及未缓解组显著较低,MTT、PS组间治疗后比较无统计学意义(P<0.05)。未缓解组治疗前后BV、BF、MTT、PS均无统计学意义(P<0.05)。结论 CT灌注成像是判定周围型肺癌放化疗疗效的有效辅助方式,灌注参数BF及BV治疗前后的变化与疗效具有良好的相关性。
Objective To study the value of CT perfusion scanning in determining the curative effect of radiotherapy and chemotherapy for peripheral lung cancer. Methods Fifty-six patients with peripheral lung cancer in our hospital were divided into remission group (n = 31) and non-remission group (n = 25) according to RECIST efficacy standard. Conclusion CT perfusion imaging is an effective adjunct to determine the efficacy of radiotherapy and chemotherapy for peripheral lung cancer. The changes of perfusion parameters BF and BV before and after treatment have a good correlation with the therapeutic effect.
