本文报导一例应用复合骨肌皮瓣一期修复口腔颌面部的巨大枪击伤的情况。病人因被枪击而致下颌骨体部、口底、舌前2/3和硬腭大部分缺损。X线显示上颌骨为LeFort Ⅰ型粉碎性骨折。伤后2天作了清创处理和气管切开术。舌的残端用断层皮片移植复盖,上颌骨骨折作了钢丝结扎。2周后从侧胸部翻起一复合皮瓣,包括部分胸大肌、20×9厘米~2的皮肤片和15厘米长的肋骨复合瓣。将肋骨结扎固定在下颌升支残端,皮瓣的尾部缝合到舌残端以修复口底。
This article reports an example of the application of composite bone and muscle flap in the first stage to repair the huge gunshot wound of oral and maxillofacial region. The patient was caused by the shot of the mandibular body, mouth bottom, tongue 2/3 and most of the hard palate defect. X-ray showed that the maxilla was a type of comminuted fracture of LeFort Ⅰ. Two days after injury, debridement and tracheotomy were performed. The stump of the tongue was covered with a cutaneous flap and the maxillary fracture was ligated. After 2 weeks from the side of the chest turned a composite flap, including part of the pectoralis major, 20 × 9 cm ~ 2 skin piece and 15 cm long rib compound flap. The rib ligament fixed in the mandibular ascending stump, the tail of the flap sutured to the tongue stump to repair the mouth.
