患者男性,64岁,住院号:68134。诉半月前无意中发现尿色较深,数天后出现血性尿,活动后更明显,但无尿痛、尿频、排尿困难等症状。 检查:营养欠佳,脸和颈部有数个蜘蛛痣,两手有明显肝掌,心肺(一),血压115/65mmHg。腹水征(一),腹壁静脉无明显怒张,肝脾未及,侧卧位可扪及约1/3左肾下极,质地正常光滑,无压痛,左肾区叩击痛(+)。左精索静脉轻度曲张。 实验室检查:血色素9.5克,血白细胞4800,血清总蛋白5克,白蛋白2克,球蛋白3克。纸上电泳:r球蛋白21.2%。尿镜检:白细胞少许,红细胞(卌),管型(一),异常细胞未找到。
