目的探讨高血压患者血压近日节律的特点与发生脑血管意外并发症的关系。方法收集发生脑出血及脑梗塞高血压患者一般资料和检测24 h动态血压,并按脑出血和脑梗塞分组,使用余弦法(包括单余弦法MSC和群体余弦法MPC)分析各自血压近日节律特点,并作统计学处理。结果使用MSC和MPC分析均发现,两组年龄、性别、血脂等一般情况无差异的状况下,其血压近日节律的相位和调整中值亦无显著差异,其节律式样的不同主要表现在脑梗塞组的血压近日节律振幅较脑出血组的血压近日节律的振幅偏低。结论血压的近日节律变化与高血压患者脑血管意外的发生相关,使用余弦分析法能够较好地评价这种近日节律变化,血压近日节律振幅高的高血压患者发生脑出血的危险较大。
Objective To investigate the relationship between the characteristics of recent rhythm of blood pressure and the occurrence of cerebrovascular accident in hypertensive patients. Methods The general data of patients with cerebral hemorrhage and cerebral infarction were collected and the ambulatory blood pressure was measured at 24 hours. According to the intracranial hemorrhage and cerebral infarction grouping, the recent rhythm characteristics of each blood pressure were analyzed by cosine method (including single cosine method and group cosine method) , And for statistical processing. Results Both MSCs and MPC analysis showed no significant difference in the phase and adjusted median of rhythm of blood pressure between the two groups in the general conditions of age, gender and blood lipid. The difference of rhythm patterns mainly manifested in cerebral infarction Group of recent rhythm of blood pressure compared with cerebral hemorrhage blood pressure rhythm of the recent low amplitude. Conclusion The recent changes of rhythm of blood pressure are associated with the occurrence of cerebrovascular accident in hypertensive patients. The use of cosine analysis can better evaluate the recent rhythm changes. There is a high risk of cerebral hemorrhage in hypertensive patients with high rhythm of blood pressure.
