为在相同条件下,评价几种环卵沉淀试验(COPT)改进方法的诊断效果,全国血吸虫病研究委员会诊断组于1983年在江苏省常熟市组织了有七个单位参加的COPT现场对比试验,并以间接血凝试验(IHA)作为对照。共试验82倒粪检阳性血吸虫病人、51例经治疗后粪检阴性病人、51名健康输血员、5例华支睾吸虫病人及4例肺吸虫病人。试验时全部血清采用双盲法。结果:几种COPT改进法的阳性检出率为65.9~85.2%。酶联环卵法(ECOPT)和荧光环卵法(FCOPT)的检测效果与常规法相比,无显著性差异:对健康人的假阳性率为2.0~33.3%,其中常规法为2.0~7.8%。常规法COPT孵育72小时的结果(69.6~84.2%)高于48小时结果(65.9~73.2%)。IHA的敏感性为91.5%,假阳性率为3.9%,但IHA对肺吸虫病人的交叉反应似较高。
In order to evaluate the diagnostic efficacy of several improved methods of ring-and-egg-precipitation test (COPT) under the same conditions, the National Schistosomiasis Research Committee Diagnostic Group organized a COPT site-controlled trial involving seven units in Changshu, Jiangsu Province in 1983, Indirect hemagglutination test (IHA) was used as a control. A total of 82 dung fecal positive schistosomiasis patients were tested, 51 post-treatment fecal negative patients, 51 healthy blood transfusion recipients, 5 clonorchiasis patients and 4 paragonimiasis patients. All sera tested double-blind method. Results: The positive rate of several improved methods of COPT was 65.9-85.2%. The detection results of ECOPT and FCOPT showed no significant difference compared with the conventional method: the false positive rate was 2.0 ~ 33.3% in healthy people, of which the routine method was 2.0 ~ 7.8% . Conventional COPT incubation for 72 hours (69.6-84.2%) was greater than 48 hours (65.9-73.2%). IHA had a sensitivity of 91.5% and a false-positive rate of 3.9%, but the cross-reactivity of IHA to paragonimiasis was higher.
