目的:探讨脑钠肽(BNP)检测对急性左心力衰竭患者的诊断价值。方法:应用荧光免疫法测定150例呼吸困难患者(心衰组112例,非心衰组38例)的血浆BNP,以心脏超声测定心脏左室射血分数(LVEF)。结果:急性心衰患者的BNP水平明显高于非心衰患者[(689±66):(45±7)];以105ng/L为正常参考值,BNP诊断心衰的敏感性为92.8%(104/112),特异性为94.74%(36/38),排除心衰的预测价值为93.3%(140/150);心衰患者的BNP水平与LVEF负相关(r=-0.60,P<0.01)。结论:应用荧光免疫法即时检测BNP,诊断心衰敏感及特异性,可作为鉴别诊断急性呼吸困难的一个观察指标。
Objective: To investigate the diagnostic value of brain natriuretic peptide (BNP) in patients with acute left heart failure. Methods: Plasma BNP was measured in 150 patients with dyspnea (112 cases of heart failure and 38 cases of non-heart failure) by fluorescence immunoassay. Cardiac left ventricular ejection fraction (LVEF) was measured by echocardiography. Results: BNP levels in patients with acute heart failure were significantly higher than those in patients with non-heart failure [(689 ± 66): 45 ± 7). The sensitivity of BNP in the diagnosis of heart failure was 92.8% at 105 ng / L 104/112), the specificity was 94.74% (36/38), and the predictive value of excluding heart failure was 93.3% (140/150). The BNP level in patients with heart failure was negatively correlated with LVEF (r = -0.60, P <0.01) ). Conclusion: The real-time detection of BNP by fluorescence immunoassay can be used as an index to diagnose acute dyspnea.
