目的:Ⅳ型胶原α5染色在Alport综合征(AS)的诊断中起重要作用,很多肾病中心对肾组织和皮肤行Ⅳ型胶原α5染色以鉴别诊断AS。本研究利用南京军区南京总医院肾脏科标本库组织比较不同抗体、不同方法间的差异。方法:选择2013年1~3月26例肾活检冰冻组织和15例皮肤石蜡组织,选用两家公司的抗体平行比较。单克隆鼠抗人Ⅳ型胶原α5抗体(Wieslab克隆号ALP105)用间接免疫荧光法,单克隆鼠抗人Ⅳ型胶原α5抗体(Cosmo BioUSA克隆号CFT-45325)用直接免疫荧光法。结果:两位病理医师随机盲看发现,两种抗体染色结果对肾脏冰冻组织诊断无差异,仅少量标本染色强度存在差别;对皮肤石蜡组织两种染色结果有较大差异。结论:Cosmo BioUSA公司抗体利用一步法省时,费用相对较低,在肾脏冰冻组织染色可替代Wieslab公司通过认证的诊断试剂盒;而皮肤石蜡组织应选择Wieslab公司试剂盒。
OBJECTIVE: Type Ⅳ collagen α5 staining plays an important role in the diagnosis of Alport syndrome (AS). Many nephrology centers stain the α5 of type Ⅳ collagen on kidney tissue and skin to differentiate AS. In this study, Nanjing University Hospital, Nanjing Military Region, the Department of Nephrology specimens were compared different antibodies, the differences between the different methods. Methods: From January to March in 2013, 26 cases of renal biopsy frozen tissues and 15 cases of skin paraffin tissues were selected for parallel comparison between the two companies. Monoclonal mouse anti-human type IV collagen alpha5 antibody (Wieslab clone No. ALP105) was detected by indirect immunofluorescence using a monoclonal mouse anti-human type IV collagen alpha5 antibody (Cosmo BioUSA clone CFT-45325) by direct immunofluorescence. Results: Two pathologists randomly found that there was no difference between the two antibody staining results in the diagnosis of kidney frozen tissue, only a small amount of specimens there is a difference in staining intensity; the two paraffin skin tissue staining results are quite different. CONCLUSIONS: Cosmo BioUSA antibodies are relatively inexpensive to use in a one-step, time-saving manner and can be used to replace Wieslab’s certified diagnostic kits in kidney frozen tissues; Wieslab’s kit should be used for skin paraffin tissue.
