目的:探讨乌司他丁联合血必净治疗高原地区与平原地区感染性休克的效果对比分析。方法:选取2014年3月—2016年5月医院收治的高原地区感染性休克患者42例为观察组,另选取同时期平原地区感染性休克患者39例为对照组,两组患者均给予乌司他丁联合血必净治疗,对比两组患者临床疗效、治疗前后PICCO测量数据:心排血指数(CI)、心输出量(CO)、血管外肺水指数(EVLWI)、体循环阻力(SVR)变化,并对比治疗前后患者白介素-6(IL-6)、降钙素原(PCT)、脑型利钠肽(BNP)水平变化。结果:观察组总有效率为59.52%,对照组患者总有效率为84.62%,与对照组对比,观察组总有效率显著降低(P<0.05);与治疗前对比,观察组与对照组患者的CI、CO、EVLWI均明显降低,SVR明显升高,且对照组患者的CI、CO、EVLWI及SVR改善程度优于观察组(P<0.05);与治疗前对比,观察组与对照组的IL-6、PCT及BNP水平均显著降低,且对照组患者的IL-6、PCT及BNP降低水平较观察组变化更显著(P<0.05)。结论:乌司他丁联合血必净治疗平原地区感染性休克的效果优于治疗高原地区感染性休克患者。
Objective: To investigate the effect of ulinastatin combined with Xuebijing in the treatment of septic shock in plateau and plain areas. Methods: From March 2014 to May 2016, 42 cases of septic shock in the plateau were selected as the observation group and 39 cases of septic shock in the same period were selected as the control group. Both groups were given Urasil He Ding combined with Xuebijing treatment, comparing the clinical curative effect of two groups of patients before and after PICCO measurement data: cardiac output (CI), cardiac output (CO), extravascular lung water index (EVLWI), systemic resistance (SVR) (IL-6), procalcitonin (PCT), brain natriuretic peptide (BNP) levels in patients before and after treatment were compared. Results: The total effective rate was 59.52% in the observation group and 84.62% in the control group. Compared with the control group, the total effective rate in the observation group decreased significantly (P <0.05). Compared with the control group, the observation group and the control group CI, CO, EVLWI were significantly lower, SVR was significantly higher, and the control group patients CI, CO, EVLWI and SVR better than the observation group (P <0.05); compared with before treatment, the observation group and the control group The levels of IL-6, PCT and BNP were significantly decreased, while the levels of IL-6, PCT and BNP in the control group were significantly lower than those in the observation group (P <0.05). Conclusion: Ulinastatin combined with Xuebijing is better than septic shock in the treatment of septic shock in the plain area.
