目的通过对北京某高校新生体检的卡痕检查、结核菌素试验、X线影像等检查项目进行分析,探讨最终确诊的活动性结核与卡痕、结核菌素试验和X线影像的相关性。方法北京某高校体检新生共5 122人,其中男1 383人,女3 739人。分为有卡痕组(A组)和无卡痕组(B组),对2组性别构成比进行比较;对所有体检者做结核菌素试验,依据试验结果将阳性学生分为阳性组、强阳性组,对2组中男女阳性率和强阳性率进行比较;所有新生均进行X线DR摄片检查,对疑诊的新生做进一步检查,共确诊活动性肺结核4人,对其PPD试验结果及卡痕进行分析。结果 A组和B组性别构成比无明显差异;PPD试验阳性者,在2组中阳性率和强阳性率比较差异无统计学意义。结论有无卡痕的性别差异无统计学意义;卡痕的有无并不能反映卡介苗接种效果的好坏;PPD强阳性并不一定是活动性肺结核;PPD阳性人群也应当纳入重点关注人群,结核的筛查要多种手段相结合。综合PPD试验和X线检查,能为高校学生提供一个防治结核的屏障,保护广大学生的健康。
OBJECTIVE: To analyze the card check, tuberculin test and X-ray examination of freshmen in a university in Beijing to investigate the correlation between the final diagnosis of active tuberculosis and card marks, tuberculin test and X-ray. Methods A total of 5,122 new medical students in a university in Beijing were enrolled, including 1 383 males and 3 739 females. Results There was no significant difference in gender composition between group A and group B. There was no significant difference in positive rate and strong positive rate between the two groups in PPD test. Conclusions There is no statistical significance in the gender differences in the presence of card marks; the presence or absence of the marks does not reflect the effect of BCG vaccination; the strong positive PPD is not necessarily active tuberculosis; PPD positive population should also be included in the focus groups, tuberculosis Screening requires a combination of means.
