目的:观察肾衰合剂对家兔急性肾盂高灌注压损伤的保护作用,研究其作用机制。方法:将30只新西兰家兔随机分为对照组、模型组及中药组,每组各10只,模型组及中药组家兔行双侧输尿管逆行插管后高压灌注,中药组给予肾衰合剂灌胃,观察各组家兔血清肌酐(SCr)、尿素氮(BUN)、内皮素(ET)、肿瘤坏死因子(TNF-α)及一氧化氮(NO)水平,测定肾小管上皮细胞钙离子浓度,并于光镜及电镜下观察肾组织变化。结果:肾衰合剂能显著降低肾盂高压灌注时家兔血SCr、BUN、ET、TNF-α水平,提高血NO水平,降低上皮细胞钙离子浓度,减轻肾小管上皮细胞破坏。结论:肾衰合剂通过改善肾血流、减轻炎性反应、拮抗钙离子释放,达到减轻肾盂高灌注压损伤的作用。
Objective: To observe the protective effects of Shenshuai Mixture on acute renal pelvic perfusion injury in rabbits and its mechanism of action. Methods: Thirty New Zealand rabbits were randomly divided into control group, model group and TCM group, with 10 rats in each group. Hyperbaric perfusion was performed in bilateral ureter retrograde intubation in model group and traditional Chinese medicine group. The levels of serum creatinine (SCr), blood urea nitrogen (BUN), endothelin (ET), tumor necrosis factor (TNF) and nitric oxide (NO) Concentration, and in light and electron microscopy renal tissue changes. Results: Shenshuai Heji could significantly decrease the blood levels of SCr, BUN, ET and TNF-α in rabbits with renal pelvic hypertension and increase the blood level of NO, decrease the concentration of calcium in epithelial cells and the destruction of renal tubular epithelial cells. Conclusion: Shenshuai Mixture can reduce renal interstitial perfusion pressure by improving renal blood flow, relieving inflammatory reaction and antagonizing calcium release.
