目的:探讨血清甘氨酰脯氨酸二肽氨基肽酶(GPDA)活性测定对胃癌的诊断价值及方法学改良。方法:速率法(酶法)检测良恶性胃病患者的GPDA活性。结果:胃癌(无转移)组25.33±6.85U/L;胃癌(肝转移)组156.92±14.65U/L;胃溃疡组76.81±11.52U/L;正常对照组78.19±10.97U/L。胃癌无转移的GPDA活性显著低于正常对照组及胃溃疡组(P<0.01),但胃癌伴肝转移者却显著高于其它各组(P<0.01)。本法线性良好,批内变异系数(CV)分别为1.98%、1.26%、2.18%;批间CV分别为2.35%、2.09%、3.18%;经对比试验相关性良好。结论:本法速度快,精度高。GPDA可作为早期胃癌诊断的指标之一,并能对胃癌的肝转移进行有效的监控。
Objective: To investigate the diagnostic value and methodological improvement of serum glycyl-proline dipeptide aminopeptidase (GPDA) activity in gastric cancer. Methods: The rate of GPDA activity in patients with benign and malignant gastropathy was measured by enzymatic method. Results: The gastric cancer (no metastasis) group was 25.33±6.85 U/L; the gastric cancer (liver metastasis) group was 156.92±14.65 U/L; the gastric ulcer group was 76.81±11.52 U/L; the normal control group 78.19±10.97 U/L. The GPDA activity of gastric cancer without metastasis was significantly lower than that in the normal control group and the gastric ulcer group (P<0.01), but the gastric cancer with liver metastasis was significantly higher than other groups (P<0.01). This method has a good linearity, with intra-assay coefficient of variation (CV) of 1.98%, 1.26%, and 2.18%, inter-assay CV of 2.35%, 2.09%, and 3.18%, respectively; The comparative test has good correlation. Conclusion: This method is fast and accurate. GPDA can be used as one of the indicators of early gastric cancer diagnosis, and can effectively monitor the liver metastasis of gastric cancer.
