目的 探讨肝内胆管癌中丙型肝炎病毒 (HCV )感染与bcl 2基因异常的关系。方法 采用半嵌套式原位聚合酶链反应和免疫组织化学方法对 2 9例原发性肝内胆管癌中bcl 2 /JH融合基因、bcl 2蛋白及HCV抗原 (NS5抗原 )进行原位观察。结果2 9例胆管癌中bcl 2 /JH融合基因均阴性 ,2 0例bcl 2蛋白阳性 ,阳性率为 69.0 %。bcl 2蛋白表达与bcl 2基因重排无相互对应关系。癌组织中NS5阳性率为 5 1.7% ( 15 / 2 9)。 15例NS5阳性中bcl 2蛋白阳性 11例 ,14例NS5阴性中bcl 2蛋白阳性 9例。 2组比较无显著性差异 (P >0 .0 5 )。结论 肝胆管上皮癌变过程中可能涉及bcl 2蛋白的参与 ,而与bcl 2 /JH融合基因无关 ,HCV感染对bcl 2蛋白表达可能无明显影响。
Objective To investigate the relationship between hepatitis C virus (HCV) infection and bcl 2 gene abnormality in intrahepatic cholangiocarcinoma. Methods The bcl 2 / JH fusion gene, bcl 2 protein and HCV antigen (NS5 antigen) in 29 patients with primary intrahepatic cholangiocarcinoma were observed by using semi-nested in situ polymerase chain reaction and immunohistochemistry . Results The bcl 2 / JH fusion gene was negative in 29 cases of cholangiocarcinoma and 20 cases of bcl 2 protein, the positive rate was 69.0%. There is no correlation between bcl 2 protein expression and bcl 2 gene rearrangement. The positive rate of NS5 in cancer tissues was 57.7% (15/29). Eleven cases were positive for bcl 2 protein in 15 cases of NS5 positive and 9 cases were positive for bcl 2 protein in 14 cases of NS5 negative. There was no significant difference between the two groups (P> 0.05). Conclusion The bcl-2 protein may be involved in the carcinogenesis of hepatobiliary ductal epithelium, but not with the bcl 2 / JH fusion gene. HCV infection may not affect the expression of bcl-2 protein.
