脑室内出血常发生在孕龄小于32周的早产儿.许多学者认为与毛细血管的末成熟有关.该文电镜观察了两例早产儿侧脑室脉络丛和脑室壁的标本.用抗IV型胶原和laminin抗体标记10例死于脑室内出血的早产儿和10例死于非脑室内出血的早产儿脉络丛和室壁的标本.见到毛细血管无完整的基膜,既有不连续的基膜样物质.IV型胶原和laminin阳性产物呈不连续线状.脑室内出血组的IV型胶原和laminin阳性产物的平均面积分别比非脑室内出血组的小,t检验P<0.05.侧脑室脉络丛和脑室壁毛细血管基膜发育落后于脑实质的基它部位,既有不连续的基模样物质和较少的IV型胶原及laminin,是侧脑室毛细血管未成熟的形态学特征之一,和脑室内出血易发生的早产儿的原因之一.
