目的探讨门诊药房退药的原因,并且提出相应的改进措施,以减少退药情况的发生。方法选择高淳人民医院药剂科2016年6—12月门诊退药的122例处方进行回顾性分析,并且提出相应的改进措施。结果导致患者退药的原因有很多种,其中由患者自身主动退药的有48张处方,占39.3%;由医院医护人员的原因所造成退药的处分有35张,占28.7%;由收费处的原因导致退药的处方有22张,占18.0%;由药品因素造成退药的处方有17张,占13.9%。在采取相应的改进措施后,药房退药的情况得到了明显的改善,退药人数不断减少。结论门诊药房出现退药的情况其实都是客观存在,有很多因素导致患者退药,但是这些都是可以采取积极的预防措施避免的,因此,医院门诊药房应不断加强管理,完善退药相关制度,提高门诊工作人员的综合素质,增强责任心,对收费处的工作人员加强各项知识的培训,以减少门诊药房的退药率。
Objective To explore the reason of withdrawal from outpatient pharmacy and propose corresponding improvement measures to reduce the occurrence of withdrawal. Methods A total of 122 prescriptions from pharmacy department of Gaochun People’s Hospital from June to December in 2016 were retrospectively analyzed and the corresponding improvement measures were put forward. As a result, there are many reasons for the patients to withdraw their medicine. Among them, 48 prescriptions are taken by the patients themselves, accounting for 39.3%; 35 cases are caused by hospital medical staffs, accounting for 28.7% of the total; There were 22 prescriptions resulting in retreat, accounting for 18.0%. There were 17 prescriptions resulting in drug withdrawal, accounting for 13.9%. After taking the corresponding improvement measures, the withdrawal situation of pharmacies has been significantly improved, and the number of drug withdrawal has been declining. Conclusions The withdrawal of drug in outpatient pharmacy is in fact an objective reality. There are many factors that lead to the withdrawal of patients. However, these can be avoided with positive preventive measures. Therefore, the outpatient pharmacy should continuously strengthen management and improve the relevant system of withdrawal , Improve the comprehensive quality of out-patient staff, enhance the sense of responsibility, strengthen staff training at the toll collection office to reduce the withdrawal rate of out-patient pharmacies.
