虽然作者发现Rose等人的48小时ACTH试验是极好的诊断方法,但本文报导3例女性患者在将ACTH加入5%葡萄糖溶液中输注时产生严重的低钠血症,而且采用地塞米松治疗也无效。Rose等已证实此问题并建议在做此试验时加以注意。可是,通常的教科书未提及静脉输入时应用何种液体或推荐用葡萄糖溶液。有些药理学文献则推荐用5%葡萄糖加水溶液。 3例患者年龄分别为46岁、31岁、35岁,一例患甲状腺功能减退,一例患肾小球肾炎,一例患阿狄森氏病,均用ACTH40U,加到5%葡萄糖500ml中持续输注12小时,48小时内重复输注,或直至试验结束为止。典型病例:女性46岁,主诉疲乏无力1年,体重减轻27kg,厌食、(?)心、呕吐、体位性头晕,色
Although the authors found Rose et al’s 48-hour ACTH test to be an excellent diagnostic, it is reported herein that 3 female patients develop severe hyponatremia when infusion of ACTH into a 5% glucose solution and that dexamethasone Treatment is also ineffective. Rose et al. Have confirmed this problem and recommended caution when doing this test. However, the usual textbooks do not mention what fluids should be used for intravenous infusion or to recommend glucose solutions. Some pharmacological literature recommends 5% dextrose in water. Three patients were 46 years old, 31 years old, 35 years old, one case of hypothyroidism, one case of glomerulonephritis, one case of Addison’s disease, were treated with ACTH40U, added to 500ml of 5% glucose continuous infusion 12 hours, repeated infusion within 48 hours, or until the end of the trial. Typical Cases: Female 46 years old, complained of fatigue and weakness for 1 year, weight loss 27kg, anorexia, (?) Heart, vomiting, positional dizziness, color
