本文对84例风湿性心瓣膜病患者心肌超微结构进行了观察,心肌超微结构损害程度达Ⅰ级27例、Ⅱ级20例、Ⅲ级17例。经直线相关分析,心肌超微结构改变与病程、心衰史、心房纤颤有显著正相关关系,与NYHA心功能分级无相关性。作者认为,NYHA心功能分级尚不能反映心肌超微结构损害程度。风湿性心瓣膜病病人出现心衰、心房纤颤,且靠强心利尿剂维持心脏代偿功能,提示心肌已有明显病理损害表现,应及早手术治疗。
In this paper, 84 patients with rheumatic valvular heart disease myocardial ultrastructure was observed, myocardial ultrastructure damage in grade Ⅰ 27 cases, Ⅱ 20 cases, Ⅲ 17 cases. Linear correlation analysis, myocardial ultrastructure changes and duration, history of heart failure, atrial fibrillation has a significant positive correlation, and NYHA functional class had no correlation. The authors believe that, NYHA cardiac function classification still can not reflect the extent of myocardial ultrastructure damage. Patients with rheumatic valvular heart failure, atrial fibrillation, and by cardiac diuretics to maintain cardiac compensatory function, suggesting that myocardial manifestations of significant pathological damage should be treated as soon as possible.
