[目的]探讨磁共振引导氩氦靶向冷冻消融治疗兔脑肿瘤的安全与风险。[方法]以经颅骨穿刺的方法将兔VX2肝癌细胞株植入28只兔右顶叶,制作兔脑肿瘤模型。饲养6d后28只兔行高场MRI扫描,将影像判断肿瘤生长良好的18只瘤兔作为治疗组,肿瘤生长不良的10只兔瘤作为对照组,治疗组行冷冻消融治疗,对照组行探针置入,未冷冻消融。术后瞬间、3d、7d,对治疗组瘤兔进行MRI扫描后分别处死2只进行病理分析,14d处死剩余试验兔进行病理分析,术后3d、7d、14d对对照组瘤兔行MRI扫描。[结果 ]治疗组冷冻消融术后病变周围可见出现水肿带,均有高颅压表现;局灶性癫痫发作6例;肢体运动障碍不能行走3例;肢体运动障碍可以行走5例;术后3天达到高峰,3天后逐渐消退,术后14天可见水肿带完全消退。病理检查结果显示术后肿瘤组织开始坏死,3d及7d部分肿瘤组织残留,14d肿瘤组织少量残留。[结论]MRI引导兔脑肿瘤氩氦靶向冷冻消融治疗有一定效果,但并发症需关注。
[Objective] To investigate the safety and risk of magnetic resonance guided argon-helium cryoablation in the treatment of brain tumors in rabbits. [Method] The rabbit VX2 hepatocarcinoma cell line was transplanted into the right parietal lobe of 28 rabbits by transcranial puncture. Rabbit brain tumor model was made. The high-field MRI scans of 28 rabbits after feeding for 6 days were used as the control group. Eighteen tumor rabbits with good tumor growth were selected as the treatment group and 10 rabbits with poor tumor growth as the control group. The treatment group was treated by cryoablation and the control group Needle into, not frozen and ablated. The rabbits in the treatment group were sacrificed on the 3rd, 7th day after the operation. Two rabbits were sacrificed for pathological analysis. The remaining rabbits were sacrificed on the 14th day for pathological analysis. The rabbits in the control group were scanned by MRI at 3d, 7d and 14d after operation. [Results] There were 6 cases of focal epilepsy in the treatment group, 6 cases of focal seizures, 3 cases of limb movement disorders can not walk, 5 cases of limb movement disorders can walk, 3 cases of postoperative 3 Days to peak, 3 days gradually subsided, 14 days after the edema zone was completely subsided. Pathological examination showed that the tumor tissue began to necrosis, 3d and 7d part of the residual tumor tissue, a small amount of residual tumor tissue 14d. [Conclusion] Targeted cryogenic ablation of argon-helium in MRI-guided rabbit brain tumors has some effect, but complications need attention.
