目的 :为制定先天性心脏病一级预防措施提供科学的参考依据。方法 :对 5 10例先天性心脏病 ( CHD)患儿进行 1∶2病例对照配对调查 ,其中室间隔缺损、房间隔缺损、法乐氏综合征和动脉导管未闭 4种主要分型 CHD各 10 2例 ,其他分型CHD共 10 2例。在建立总的不分型 CHD与各危险因素的条件 L ogistic模型并筛选出 CHD总的危险因素之后 ,采用多个因变量和多个自变量的条件 L ogistic模型 ,对各分型 CHD与可疑危险因素进行多因多果分析 ,分别筛选出各分型 CHD相应的危险因素。结果 :不分型 CHD总的危险因素与各分型 CHD的危险因素不尽相同 ,各分型 CHD之间的危险因素也有差别。结论 :制定 CHD一级预防措施仅依据不分型总的病因研究结论是不全面的 ,还应考虑各分型 CHD相应危险因素的研究结论
Objective: To provide a scientific reference for the development of primary preventive measures for congenital heart disease. Methods: A total of 510 children with congenital heart disease (CHD) were enrolled in a 1: 2 case-control study. Four major subtypes of ventricular septal defect, atrial septal defect, Farrell’s syndrome and patent ductus arteriosus 10 2 cases, other subtypes of CHD a total of 102 cases. After establishing general L ogistic model of undifferentiated CHD and various risk factors and screening out the total risk factors of CHD, the conditional L ogistic model with multiple dependent variables and multiple independent variables was used to analyze the relationship between each type of CHD and suspicious Risk factors for multi-factor multi-analysis, were screened for each type of CHD corresponding risk factors. Results: The overall risk factors of undifferentiated CHD were different from the risk factors of each type of CHD, and the risk factors of CHD were also different. Conclusion: It is not comprehensive to make a primary prevention measure of CHD based on the total type of etiology, and the conclusions of the research on the corresponding risk factors for each subtype of CHD
