目的评价超声图像作为高强度聚焦超声治疗子宫肌瘤时作用及地位。方法利用高强度聚焦超声治疗子宫肌瘤患者16例,观察子宫肌瘤术中及术后超声图像的变化,同时对照手术前后核磁共振(MRI)的动态变化。结果HIFU治疗时,可见超声灰阶增强,术后随访肌瘤血流减少或消失,术后MRI显示治疗靶区坏死明显,坏死区与超声定位治疗靶区相一致,提示超声定位及监控准确。周围邻近脏器未见损伤。结论超声定位是目前高强度聚焦超声治疗子宫肌瘤较好的定位及治疗监控手段。
Objective To evaluate the role and status of ultrasound in the treatment of uterine fibroids by high intensity focused ultrasound. Methods High intensity focused ultrasound was used to treat 16 cases of uterine fibroids. The changes of intraoperative and postoperative ultrasound images of uterine fibroids were observed, and the changes of MRI before and after the operation were compared. Results When HIFU was treated, the gray scale of the ultrasound was enhanced. The blood flow of the fibroids was reduced or disappeared after the operation. The postoperative MRI showed that the necrosis of the target area was obvious. The necrotic area was consistent with the target area of ultrasound localization treatment, which showed that ultrasound localization and monitoring were accurate. No injuries were found in nearby organs. Conclusion Ultrasound localization is the better positioning and treatment monitoring method of high-intensity focused ultrasound for uterine fibroids.
