目的探讨急性砷中毒引发心律失常的原因及对心脏传导系统的毒性作用。方法回顾性分析29例急性砷中毒患者临床资料,按心律失常与否分为心律失常组和心律正常组进行心律失常类型和尿砷水平比较分析。结果29例急性砷中毒病例有7例出现心律失常,其中4例为恶性心律失常,心律失常组尿砷水平(中位数为204.74μmol/L)普遍高于心律正常组(中位数为74.54μmol/L),2组比较,差异有统计学意义(P=0.006)。结论尿砷水平越高,发生心律失常的风险越高。严重砷中毒病例应提早预防恶性心律失常的发生。
Objective To investigate the causes of arrhythmia caused by acute arsenism and the toxic effects on cardiac conduction system. Methods The clinical data of 29 patients with acute arsenism were retrospectively analyzed. The types of arrhythmia and urinary arsenic levels were analyzed by arrhythmia group and arrhythmia group. Results Twenty-nine patients with acute arsenism had arrhythmia, of which 4 were malignant arrhythmia. The urinary arsenic level in arrhythmia group (median 204.74μmol / L) was generally higher than that in normal heart rate group (median 74.54 μmol / L). The difference between the two groups was statistically significant (P = 0.006). Conclusion The higher the level of urinary arsenic, the higher the risk of arrhythmia. Severe arsenic poisoning cases should be the early prevention of malignant arrhythmia.
