目的评价国产冷冻消融设备在≤5 cm肝细胞肝癌(hepatocellular carcinoma,HCC)消融治疗的安全性及有效性.方法对33例直径≤5 cm HCC,采用国产靶向刀行计算机断层扫描引导下冷冻消融治疗,评估术中、术后手术相关并发症,通过影像学复查评价目标病灶消融效果及术后局部复发率.结果全部33例(共37个病灶)均成功行冷冻消融治疗,消融结束时,消融靶区完全覆盖病灶率为83.78%(31/37),部分覆盖或消融区域达到病灶边缘率为16.22%(6/37).术中、术后无严重并发症出现.平均10.8 mo随访中,32例存活,1例失访,肿瘤病灶局部复发率30.3%(10/33),治疗效果与进口设备相当.结论本研究揭示该国产冷冻消融系统在≤5 cm HCC消融治疗中安全有效,能有效灭活局部瘤体.
Objective To evaluate the safety and efficacy of home-made cryoablation ablation in the treatment of hepatocellular carcinoma (HCC) ≤5 cm.Methods Thirty-three patients with diameter ≤5 cm HCC were treated with domestic target-knife computed tomography Ablation therapy to evaluate intraoperative and postoperative complications related to surgery, evaluation of the target lesion ablation and postoperative local recurrence rate by imaging review.Results All 33 cases (37 lesions) were successfully treated by cryoablation and ablation (83.78%, 31/37), and the marginal rate of lesion in partially covered or ablated area was 16.22% (6/37) .There was no serious complications during and after surgery.The average follow-up of 10.8 months , 32 patients survived and 1 patient was lost.The local recurrence rate of tumor lesions was 30.3% (10/33), and the therapeutic effect was similar to that of imported equipment.Conclusions This study shows that the domestic cryoablation system is safe and effective in ablation of ≤5 cm HCC , Can effectively inactivate local tumor.
