我院于1985～1988年收治3例上颌窦纤维组织细胞瘤病人,死亡2例,失访1例。现举1例报道如下。 患者女,41岁。左上牙痛8个月,左面部麻木及头痛1个月,在当地医院拔除左上磨牙后疼痛不缓解而行上颌窦手术(无病理),术后症状不减而来本院。检查:左上牙龈肿胀,硬腭左侧下陷。X线片示左上颌窦密度增高,外壁破坏,左翼突内、外板及卵圆孔、棘孔不清。全麻下行上颌骨截除、眶内容摘除,术中见肿瘤广泛破坏上颌窦壁,侵及翼腭窝和颞下窝。病理诊断为恶性纤维组织细胞瘤。术后病人仍有头痛,5个月后死于脑转移。
In our hospital from 1985 to 1988, 3 patients with maxillary sinusitis were treated, 2 patients died and 1 patient was lost. One case is reported below. Female patient, 41 years old. Upper left toothache 8 months left numbness and headache for 1 month in the local hospital after the removal of the left upper molars pain does not relieve the line maxillary sinus surgery (no pathology), postoperative symptoms diminished come to our hospital. Check: The upper left gingival swelling, left palate hard subsidence. X-ray showed increased density of the left maxillary sinus wall destruction, the left wing, the outer plate and foramen ovale, spine holes unclear. Under general anesthesia maxillary excision, removal of orbital contents, intraoperative see the tumor extensive destruction of the maxillary sinus wall, invasion of the pterygopalatine fossa and infratemporal fossa. Pathological diagnosis of malignant fibrous histiocytoma. The patient still had headache after the operation and died of brain metastasis 5 months later.
