目的寻求快速、简便、准确的布鲁杆菌病临床诊断方法。方法采用布鲁杆菌病虎红平板凝集试验(RBPT)和试管凝集试验(SAT),对疑似布鲁杆菌病患者血清进行同步检验。结果用RBPT和SAT 2种方法同时检测疑似布病患者血清524份,RBPT的阳性率为21.18%(111/524),SAT的阳性率为22.14%(116/524)。在413份RBPT阴性的血清中,有7份SAT阳性,假阳性率为1.69%。RBPT与SAT总体阳性进行比较,敏感度为93.97%,特异度为99.51%。经一致性检验,K值为0.95,2种试验方法的一致性很高;RBPT与SAT 1∶100++及以上阳性者比较,K值为0.60,2种试验方法的一致性不高。结论 RBPT的假阳性率很高,不能替代SAT用于布病临床诊断。
Objective To seek fast, simple and accurate clinical diagnosis of brucellosis. Methods The Brucella rhubarb plate agglutination test (RBPT) and the test tube agglutination test (SAT) were used to test the serums of suspected brucellosis patients. Results 524 serums of patients with suspected brucellosis were detected simultaneously by RBPT and SAT. The positive rate of RBPT was 21.18% (111/524) and the positive rate of SAT was 22.14% (116/524). Of the 413 RBPT-negative sera, 7 were SAT-positive with a false-positive rate of 1.69%. RBPT and SAT overall positive comparison, the sensitivity was 93.97%, the specificity was 99.51%. The concordance test showed that the K value was 0.95, and the consistency of the two test methods was very high. The K value was 0.60 for both RBPT and SAT 1: 100 ++ positive and above, but the consistency of the two test methods was not high. Conclusion RBPT false positive rate is very high, can not replace the SAT for clinical diagnosis of brucellosis.
