本文观察了环氧酶抑制剂消炎痛对全胃切除术引起中等程度创伤的应激反应和蛋白质分解代谢的影响。消炎痛组从手术结束后2h给予消炎痛肛栓剂,50mg,1次/8h,直至术后第六天,对照组按同样方法给予安慰剂。监测了手术前后体温、血浆蛋白、与应激相关的内分泌激素和尿中尿素氮、肌酐和3-MH的变化。结果显示消炎痛组比对照组体温平均减低0.5℃左右,血中去甲肾上腺素和皮质醇明显低于对照组,累积尿中尿素氮和3-MH排出比对照组显著减少,但血浆蛋白在两组间差异不明显。说明环氧酶抑制剂能够明显减轻创伤病人的应激反应和蛋白质分解代谢。
This article investigates the effect of the cyclooxygenase inhibitor indomethacin on the stress response and protein catabolism caused by total gastrectomy. The indomethacin group was given indomethacin suppository 2h after the operation, 50mg once a day for 8h until the sixth day after the operation. The control group was given placebo in the same way. Body temperature, plasma proteins, stress-related endocrine hormones, and urinary nitrogen, creatinine, and 3-MH levels were monitored before and after surgery. The results showed that body temperature in indomethacin group was reduced by about 0.5 ℃ on average, blood norepinephrine and cortisol were significantly lower than those in control group, urea nitrogen and 3-MH excretion in urine were significantly lower than those in control group, but plasma The difference between the two groups was not obvious. Epoxide enzyme inhibitors can significantly reduce stress response and protein catabolism in trauma patients.
