目的:探讨使用荧光原位杂交技术检测乳腺原发癌和淋巴结转移癌HER-2基因的表达及意义。方法:选取我院2013年6月至2014年6月收治的乳腺原发癌患者133例作为研究对象,对其临床资料进行回顾性分析。应用荧光原位杂交技术(FISH)与免疫组化技术(IHC)对所有患者手术切除标本的HER-2基因表达予以检测。结果:FISH阳性率为42.1%,IHC为58.6%;IHC阴性时与FISH符合率为87.2%,IHC(+)与FISH符合率为34.7%,IHC(++)与FISH符合率为83.8%,IHC(+++)与FISH符合率为100.0%。淋巴结转移阳性率为67.5%,未转移阳性率为33.3%,组间比较,差异具有统计学意义(P<0.05)。结论:IHC对HER-2基因予以检测时若结果为阴性或者强阳性则与FISH符合率较高,同时HER-2基因表达与腋淋巴结转移之间存在正相关关系。
Objective: To investigate the expression and significance of HER-2 gene in primary breast cancer and lymph node metastatic carcinoma using fluorescence in situ hybridization. Methods: A total of 133 patients with primary breast cancer who were treated in our hospital from June 2013 to June 2014 were selected as the research object, and their clinical data were retrospectively analyzed. The HER-2 gene expression in all patients with surgically resected specimens was detected by fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC). Results: The positive rate of FISH was 42.1%, IHC was 58.6%. The coincidence rate of IHC (+) and FISH was 37.2% when ICH was negative, the coincidence rate of IHC (+) and FISH was 34.7% The coincidence rate of IHC (+++) and FISH was 100.0%. The positive rate of lymph node metastasis was 67.5%, the positive rate of non-metastasis was 33.3%. There was significant difference between the two groups (P <0.05). CONCLUSION: IHC has a high coincidence rate with FISH if the result of HER-2 gene is negative or strong, and there is a positive correlation between HER-2 gene expression and axillary lymph node metastasis.
