评价胃癌根治切除并用温热重蒸馏水 (DoubleDistilledWater ,DDW)或DDW加醋酸洗必泰腹腔灌洗的疗效与适应证。方法 在进行了 43℃DDW对人胃癌细胞系MGC 80 3的杀伤效应与SY86B腹水瘤受抑制的研究基础上 ,将 1986年 1月～ 1995年 12月施行胃癌根治切除术的 5 0 0例病人分成 3组。A组 :根治术并用 43℃DDW40 0 0ml腹腔灌洗 10min ,共 198例 ;B组 :根治术并用DDW40 0 0ml+醋酸洗必泰 0 .6g ,腹腔灌洗 4min ,共 89例 ;C组 :根治术关腹前应用生理盐水 40 0 0ml,腹腔清洗 4min ,共 2 13例。结果 实验证明 ,应用 43℃DDW对人胃癌细胞系MGC 80 3处理 10min ,或用 0 .0 15 %醋酸洗必泰DDW处理 4min ,癌细胞可被杀灭。临床应用结果表明 ,A组与B组疗效相同 ,二者合在一起计算称灌洗组。各期病例术后 1、3年生存率 ,3组间差异无显著意义 ;全组 5年生存率 ,灌洗组为 6 3 .8% ,对照组为 5 1.2 %。而且有效病例多为灌洗组的中期 (Ⅱ、ⅢA)病例。结论 胃癌根治术 ,关腹前采用本疗法行腹腔灌洗 ,对Ⅱ期、ⅢA期病例有良好效果。
To evaluate the curative effect and indications of radical resection of gastric cancer with double distilled water (DDW) or DDW plus chlorhexidine acetate peritoneal lavage. Methods Based on the study of the killing effect of DDW on human gastric cancer cell line MGC 80 3 and the inhibition of SY86B ascites tumor at 43 ℃, 500 patients undergoing radical resection of gastric cancer from January 1986 to December 1995 were enrolled in this study. Divided into 3 groups. Group A: radical mastectomy with 43 ℃ DDW40 0ml intraperitoneal lavage 10min, a total of 198 cases; B group: radical mastectomy and DDW40 0ml + chlorhexidine acetate 0.6g, peritoneal lavage 4min, a total of 89 cases; C group: radical Applied arteries closed 40000 saline, abdominal washing 4min, a total of 213 cases. Results The experiment proved that the application of DDW at 43 ℃ for 10 min in human gastric cancer cell line MGC 80 3 or treated with 0.15% chlorhexidine acetate for 4 min resulted in the killing of cancer cells. Clinical application results show that, A group and B group the same efficacy, the two together to calculate that lavage group. The 1-year and 3-year survival rates were not significantly different among the three groups. The overall 5-year overall survival rate was 63.8% in the lavage group and 5 1.2% in the control group. And effective cases and more for the mid-lavage group (Ⅱ, Ⅲ A) cases. Conclusions Radical operation of gastric cancer and peritoneal lavage with this therapy before closing the stomach have good effect on stage Ⅱ and ⅢA.
