原发性心脏滑膜肉瘤的临床病理学观察

目的:探讨原发性心脏滑膜肉瘤(synovial sarcoma,SS)的病理形态学和免疫表型特点及其鉴别诊断和预后评估。方法:在光学显微镜下观察1例心脏SS患者的肿瘤标本,并进行免疫组织化学染色和荧光原位杂交(fluorescence in situ hybridization,FISH)检测,结合文献分析讨论。结果:SS由单一的梭形细胞组成,呈交织束状排列,部分间质黏液变性。免疫组织化学检测结果示,AE1/AE3(部分+),CK7(散在+),CK19(散在+),波形蛋白(vimentin)(+),bcl-2(+),Calponin(+);FISH检测结果示,18号染色体基因位点断裂及易位。结论:心脏单相型SS诊断较困难,易误诊,需结合形态改变、免疫表型与分子生物学检测结果判断。 Objective: To investigate the pathomorphology and immunophenotype of primary synovial sarcoma (SS) and to evaluate its differential diagnosis and prognosis. Methods: The tumor specimens of one patient with cardiac SS were observed under a light microscope. Immunohistochemistry and FISH were used to detect the tumor samples, which were analyzed and discussed in literature. Results: The SS consisted of a single spindle cell, arranged in the form of intertwined bundles with some interstitial mucus degeneration. The results of immunohistochemistry showed that there was no significant difference in the expression of AE1 / AE3 (partial +), CK7 (scattered +), CK19 (scattered +), vimentin (+), bcl- The results showed that chromosome 18 locus was broken and translocated. Conclusion: Single heart SS diagnosis is difficult and easily misdiagnosed. It needs to be combined with morphological changes, immunophenotype and molecular biological test results.
