目的探索一种对眼部恶性肿瘤较可靠实用的诊断和随诊方法。方法对眼部恶性肿瘤患者47例用RIA法测定了泪液和血清β2-微球蛋白(β2-M)、癌胚抗原(CEA)的含量,以良性肿瘤44例和正常人50例对照。结果恶性肿瘤β2-M含量,泪液14.61±5.57mg/L、血清1.54±1.08mg/L,另两组泪液β2-M含量均值分别为8.60±2.62mg/L,8.54±1.40mg/L.升高阳性率泪液71.11%、血清9.52%.恶性肿瘤组CEA含量,泪液83.24±47.26μg/L,血清11.11±5.25μg/L,另两组泪液CEA含量分别为24.26±9.92μg/L,20.17±3.46μg/L.升高阳性率泪液95.35%、血清4.76%.泪液β2-M和CEA值均升高的阳性率69.05%.恶性肿瘤组与另两组比较,差异非常显著(P<0.01),升高阳性率也明显高于血清。术后观察无复性者泪液两项值下降至正常,有复发者升高。结论泪液和血清β2-Μ,CEA检测并结合临床可作为眼部恶性肿瘤的一项较可靠的辅助诊断指标和随诊方法,有较大的临床应用价值。
Objective To explore a more reliable and practical method for the diagnosis and follow-up of ocular malignant tumors. Methods The contents of tear and serum β2-microglobulin (β2-M) and carcinoembryonic antigen (CEA) in 47 patients with ocular malignant tumors were determined by RIA method. The results were compared with 44 cases of benign tumors and 50 normal controls. Results The β2-M content of malignant tumor was 14.61 ± 5.57mg / L for tear and 1.54 ± 1.08mg / L for serum. The mean β2-M contents in the other two groups were 8.60 ± 2.62mg / L , 8.54 ± 1.40 mg / L. Elevated positive rate of tear 71.11%, serum 9.52%. CEA content in malignant tumor group was 83.24 ± 47.26μg / L in tear and 11.11 ± 5.25μg / L in serum, CEA contents in the other two groups were 24.26 ± 9.92μg / L and 20.17 ± 3.46 μg / L. Increased positive rate of 95.35% tear, serum 4.76%. The positive rate of tear β2-M and CEA were 69.05%. There was a significant difference between the malignant tumor group and the other two groups (P <0.01), and the positive rate was also significantly higher than that of the serum. After the observation of non-reflex tear drops two values to normal, with recurrence increased. Conclusion Laccase, serum β2-M, CEA combined with clinical diagnosis can be used as a more reliable diagnostic indicator and follow-up method for ocular malignant tumors, which has great clinical value.
