目的贯彻执行《处方管理办法》,动态分析医院门诊处方存在的主要问题,以期寻找持续改进措施,提高处方质量,促进临床合理用药。方法随机抽取2007年5月至2008年4月每月门诊处方400张和2009年5月至2010年4月每月门诊处方600张,合计分别为4800张和7200张。按照《处方管理办法》的要求逐张审查,记录每张处方存在的问题,分析处方不合理用药情况,分别统计各种不合理原因在处方中出现的比例。结果 2007年5月至2008年4月有不合格处方3556张,占抽查处方的74.08%;2009年5月至2010年4月有不合格处方1435张,占抽查处方的19.93%。不合格原因主要有处方书写不规范和用药不适宜两方面。结论使用药品商品名、急三慢七、抗菌药物的不合理应用是医院处方存在的主要问题,如何持续改进和提高处方质量,任务繁重。
Objective To implement the “prescription management approach” to dynamically analyze the main problems existing in hospital outpatient prescriptions in order to find continuous improvement measures to improve the quality of prescriptions and promote the rational use of clinical medicine. Methods From May 2007 to April 2008, 400 outpatient prescriptions per month and 600 outpatient prescriptions per month from May 2009 to April 2010 were collected randomly, totaling 4800 and 7200 respectively. According to the requirements of the “prescription management approach” one by one to review and record the problems of each prescription, analysis of unreasonable use of prescriptions were statistics of various unreasonable reasons in the proportion of the prescription. Results There were 3,556 unqualified prescriptions from May 2007 to April 2008, accounting for 74.08% of the random prescriptions. From May 2009 to April 2010, there were 1,435 unqualified prescriptions, accounting for 19.93% of the spot checks. The main reasons for the unqualified prescription writing and medication inappropriate two aspects. Conclusions The unreasonable use of antibacterial drugs is the main problem in hospital prescriptions when the drug product name is used. The application of the drug product name, JI SAN JI JI, and so on, how to continuously improve and enhance the quality of prescriptions is a heavy task.
