目的:探讨社区护理干预对冠心病合并高脂血症患者的效果。方法:选择连南县寨岗镇卫生院2014年1月至2015年1月门诊接收的老年冠心病合并高脂血症患者90例,随机分为对照组和观察组各为45例,对照组接受常规指导,观察组在常规指导的基础上,给予患者有针对性的社区护理干预,观察和比较两组患者的心绞痛发作次数/周、发作时间、硝酸甘油变化/周、血压、心率、治疗依从性(Morisky-Green评量表)、治疗效果、干预前后自我效能(SES评量表)以及生活质量(WHO-QOL-BREF评量表)。结果:(1)与对照组对比,观察组心绞痛发作次数/周明显更少,发作时间明显更短,硝酸甘油变化/周明显更小(P<0.05);与治疗前对比,两组治疗后心绞痛发作次数/周明显减少,发作时间明显缩短,硝酸甘油变化/周明显减小,差异具有统计学意义(P<0.05)。(2)与对照组对比,观察组治疗依从性明显更高,干预后自我效能、生活质量评分明显更高,差异具有统计学意义(P<0.05)。与治疗前对比,治疗后两组患者自我效能和生活质量评分明显提高,差异具有统计学意义(P<0.05)。结论:冠心病合并高脂血症患者的社区护理干预效果显著,能够有效地控制患者的病情发展。
Objective: To investigate the effect of community nursing intervention on patients with coronary heart disease complicated with hyperlipidemia. Methods: Ninety elderly patients with coronary heart disease complicated with hyperlipidemia admitted to outpatient clinics in Zhaogang Town, Lian Gang County from January 2014 to January 2015 were randomly divided into control group and observation group, with 45 cases in each group. The control group The patients in the observation group were given routine guidance. The observation group received targeted community nursing intervention on the basis of routine guidance. The number of angina attacks per week, the onset time, the change of nitroglycerin / week, blood pressure, heart rate, treatment Compliance (Morisky-Green Scale), treatment outcome, pre-intervention self-efficacy (SES scale), and quality of life (WHO-QOL-BREF scale). Results: (1) Compared with the control group, the number of angina pectoris episodes / week was significantly less in the observation group, the onset time was significantly shorter and the change of nitroglycerin / week was significantly smaller (P <0.05) The number of angina attacks / week decreased significantly, the onset time was significantly shortened, the change of nitroglycerin / week was significantly reduced, the difference was statistically significant (P <0.05). (2) Compared with the control group, the compliance of the observation group was significantly higher, and self-efficacy and quality of life scores were significantly higher after intervention (P <0.05). Compared with those before treatment, the self-efficacy and quality of life scores of two groups were significantly improved after treatment, the difference was statistically significant (P <0.05). Conclusion: Community nursing intervention in patients with coronary heart disease complicated with hyperlipidemia is effective and can effectively control the patient’s condition development.
