目的探讨心率变化对16层螺旋CT冠状动脉成像质量的影响和减慢与稳定心率的方法与效果。方法181例患者接受16层螺旋CT冠状动脉成像术,将原始数据送AW4.1工作站,对冠状动脉作容积再现(VR)、二维曲面重建和最大密度投影(MIP)、心脏长、短轴重建等。图像质量的评价标准分能满足冠状动脉管腔评价和不能满足冠状动脉管腔评价两种。结果84例(46%)患者原来心率超过75次/min,经过采取减慢和稳定心率的综合措施后,心率降低至75次/min以下。181例患者中有150例(82.9%)的整体图像能满足冠状动脉管腔评价的要求;31例(17.1%)未能达到满足冠状动脉管腔评价的标准,其中2例(1.1%)是操作不当、4例(2.2%)是扫描过程中患者屏气不完全、25例(13.8%)是扫描过程中患者心率波动过大(变动幅度超过10次/min)所致。结论在正确选择患者的情形下,心率是影响16层螺旋CT冠状动脉成像质量的重要因素。
Objective To investigate the effect of heart rate changes on the quality of 16-slice spiral CT coronary angiography and to decrease and stabilize the heart rate. Methods 181 patients underwent 16-slice spiral CT coronary angiography. The raw data were sent to AW4.1 workstation for volumetric reconstruction of coronary artery (VR), two-dimensional surface reconstruction and maximum density projection (MIP), long and short cardiac axis Reconstruction and so on. Image quality evaluation criteria can meet the coronary artery lumen evaluation and can not meet the coronary artery lumen evaluation of two. Results In 84 patients (46%), the original heart rate was over 75 beats / min. The heart rate was reduced to less than 75 beats / min after taking comprehensive measures to slow down and stabilize the heart rate. The overall image of 150 of 181 patients (82.9%) met the requirements of the luminal evaluation of coronary arteries; 31 (17.1%) failed to meet the criteria of luminal evaluation of the coronary arteries, of which 2 (1.1%) were Inappropriate operation, 4 patients (2.2%) were incomplete breath-hold during scanning and 25 (13.8%) were due to excessive heart rate fluctuations (over 10 beats / min) during scanning. Conclusion In the correct choice of patients, heart rate is an important factor affecting the quality of 16-slice spiral CT coronary angiography.
