背景:在三叉神经痛的治疗中,传统的方法有药物、封闭、手术等。而立体定向放射治疗三叉神经痛具有无创伤、无痛苦、疗效显著等优点,对这种方法的研究国内外刚刚起步。目的:探讨立体定向放射治疗对三叉神经疼痛症状的改善效应。设计:以患者为观察对象的病例分析。单位:河南科技大学第一附属医院X 刀治疗中心。对象:选择1999-03/2002-03河南科技大学第一附属医院X 刀治疗中心收治的三叉神经痛患者30例,均经临床诊断和CT 或M RI证实,无其他疾病引起,经长期药物或手术治疗效果不佳的患者为研究对象。方法:对30例三叉神经痛患者实施头戴面罩,单次、非共面、多角度的立体定向放射治疗。靶点数2个,准直器5m m ,边周剂量60～75Gy,参考剂量曲线85% ～90%。主要观察指标:疗效评定结果和随访结果。结果:对30例患者均实施随访,随访1～22个月,12～24h 症状减轻或缓解者6例,1～3个月显效者20例,4例效果不明显,有效率达87%。1例出现患侧面部麻木,1例首次治疗效果不佳,再次调整剂量后效果显著。结论:立体定向放射治疗三叉神经痛效果肯定,无特殊副反应。
Background: In the treatment of trigeminal neuralgia, the traditional methods are medicine, occlusion, surgery and so on. Stereotactic radiotherapy of trigeminal neuralgia with non-invasive, painless, significant effect, etc., this method of research has just started. Objective: To investigate the effect of stereotactic radiotherapy on trigeminal pain. Design: case analysis of patients as the object of observation. Unit: First Affiliated Hospital of Henan University of Science and Technology X knife treatment center. PARTICIPANTS: Thirty cases of trigeminal neuralgia admitted to the First Affiliated Hospital of Henan University of Science and Technology from March 1999 to March 2002 were confirmed by clinical diagnosis and CT or M RI, without other diseases. After long-term drug or Surgical treatment of poor patients as the research object. Methods: Thirty patients with trigeminal neuralgia underwent masked, single, non-coplanar and multi-angle stereotactic radiotherapy. Target number 2, collimator 5m m, marginal dose of 60 ~ 75Gy, the reference dose curve of 85% to 90%. MAIN OUTCOME MEASURES: Efficacy evaluation results and follow-up results. Results: All 30 patients were followed up for 1 ~ 22 months. In 12 ~ 24h, 6 cases were relieved or relieved of symptoms, 20 cases were effective in 1 ~ 3 months. The effect was not obvious in 4 cases, and the effective rate was 87%. One case had numbness on the side of the face, and the first treatment was ineffective in one case. The effect was significant after adjusting the dosage again. Conclusion: The effect of stereotactic radiotherapy for trigeminal neuralgia is affirmative without any special side effects.
