目的:探讨3种肝癌标志物甲胎蛋白(alpha fetal protein,AFP)mRNA,胰岛素样生长因子-Ⅱ(insulin-likegrowth factrorⅡ,IGF-Ⅱ)mRNA和转化生长因子β1(transforming growth factorβ1,TGF-β1)mRNA在肝癌诊断中的临床价值。方法:125例肝癌患者(肝癌组)、131例良性肝病患者(良性肝病组)、32例非肝脏肿瘤患者(肝外肿瘤组)及65名健康体检者(正常对照组),提取外周血中总RNA,经随机引物和反转录酶作用分别合成AFP,IGF-Ⅱ和TGF-β1的cDNA,再以巢式PCR分别扩增其cDNA片段,分析其在肝癌诊断与鉴别中的临床价值。结果:外周血中扩增出的AFP mRNA,IGF-ⅡmRNA和TGF-β1mRNA基因片段大小为分别为159,170,211 bp,与原设计一致。肝癌组外周血AFP mRNA,IGF-ⅡmRNA和TGF-β1mRNA基因片段阳性检出率明显高于良性肝病组、肝外肿瘤组和正常对照组(P<0.05)。结论:外周血AFP mRNA,IGF-ⅡmRNA和TGF-β1mRNA基因分析有助于肝癌的早期诊断、转移判断及术后复发的监测。
Objective: To investigate the expression of three kinds of markers of hepatocellular carcinoma including alpha-fetoprotein (AFP), insulin-like growth factor-Ⅱ (IGF-Ⅱ) mRNA and transforming growth factor β1 ) mRNA in the diagnosis of liver cancer. Methods: Totally 125 cases of hepatocellular carcinoma (HCC), 131 cases of benign liver disease (benign liver disease), 32 cases of non-liver tumor (extrahepatic tumor) and 65 healthy controls (normal control group) Total RNA was synthesized by random primers and reverse transcriptase. The cDNA fragments of AFP, IGF-Ⅱ and TGF-β1 were synthesized by nested PCR. The cDNA fragments were amplified by nested PCR and their clinical value in diagnosis and differential diagnosis of HCC was analyzed. Results: The size of AFP mRNA, IGF-ⅡmRNA and TGF-β1mRNA gene fragments amplified in peripheral blood were 159,170 and 211 bp, respectively, consistent with the original design. The positive rates of AFP mRNA, IGF-ⅡmRNA and TGF-β1 mRNA in peripheral blood of hepatocellular carcinoma group were significantly higher than those in benign liver disease group, extrahepatic tumor group and normal control group (P <0.05). Conclusion: The analysis of AFP mRNA, IGF-ⅡmRNA and TGF-β1mRNA in peripheral blood is helpful for the early diagnosis, metastasis and monitoring of postoperative recurrence.
