目的:探讨自我采样检测HPV应用于宫颈病变筛查的可行性。方法:全面检索Pubmed、Embase、Cochrane library、CBM、CNKI、VIP和万方等数据库,纳入自我采样检测HPV应用于宫颈病变筛查的随机对照试验,使用Revman软件合并分析自我采样检测HPV应用于宫颈病变筛查的相关指标。结果:最终纳入12个RCT研究,共92027例女性参与宫颈病变筛查。Meta分析结果显示,自我采样检测HPV可以明显增加宫颈病变的筛查参与率(RR=2.89,95%CI为1.85~4.50);对于30~39岁以及60岁以上的人群,自我采样检测HPV可明显提高宫颈疾病的筛查参与率,而对于40~49岁以及50~59岁的人群,自我采样检测HPV却没有明显提高宫颈疾病的筛查参与率。自我采样检测HPV可以明显提高宫颈病变的检出率(RR=2.73,95%CI为1.66~4.48)。结论:自我采样检测HPV可以明显提高宫颈病变筛查的筛查参与率和宫颈病变检出率。
Objective: To explore the feasibility of using self-sampling to detect cervical lesions. METHODS: A comprehensive database of Pubmed, Embase, Cochrane library, CBM, CNKI, VIP and Wanfang databases was included in a randomized controlled trial of self-sampling to detect HPV in cervical lesions. Self-sampling was performed using Revman software to detect HPV in cervical Disease screening related indicators. RESULTS: Twelve RCTs were finally included, with a total of 92027 women participating in the screening of cervical lesions. Meta-analysis showed that self-sampling of HPV could significantly increase the screening rate of cervical lesions (RR = 2.89, 95% CI 1.85-4.50); for 30-39 years old and over 60 years of age, self-sampling HPV Significantly improve the screening rate of cervical disease participation, and for 40 to 49 years old and 50 to 59 years of age, self-sampling HPV testing did not significantly improve the screening rate of cervical disease participation. Self-sampling detection of HPV can significantly improve the detection rate of cervical lesions (RR = 2.73, 95% CI 1.66 ~ 4.48). Conclusion: Self-sampling HPV testing can significantly improve the cervical screening screening rate of screening and cervical lesions detection rate.
