目的阐述磁共振评价肝细胞癌微血管侵犯的研究进展,为临床肝细胞癌患者治疗方案选择提供相关信息。方法检索Peb Med、OVID、CNKI等数据库,以“肝细胞癌”“微血管侵犯”“磁共振”为关键词,检索1950年5月到2017年5月的相关文献,共检索英文文献67篇,中文文献13篇。纳入标准为:(1)影像学方法为磁共振;(2)评价内容为肝细胞癌微血管侵犯;(3)金标准为术后病理结果。最终54篇文献纳入本综述分析。结果目前磁共振用于评价肝细胞癌微血管侵犯的方法较多,常用的方法包括形态学评价、纹理分析、弥散加权、动态增强、脂肪分析、肝细胞功能成像及综合分析。结论各种磁共振方法在评价微血管侵犯方面表现不一,多模态磁共振联合应用在综合预测肝细胞癌的微血管侵犯方面有巨大潜力。
Objective To describe the research progress of MRI in the evaluation of microvascular invasion of hepatocellular carcinoma (HCC) and to provide relevant information for the selection of treatment options in patients with HCC. Methods The databases of Peb Med, OVID and CNKI were searched and the key words “Hepatocellular carcinoma”, “Microvascular invasion” and “Magnetic resonance imaging” were searched, and the relevant documents from May 1950 to May 2017 were retrieved. Retrieved 67 English articles and 13 Chinese articles. Inclusion criteria: (1) imaging method for magnetic resonance; (2) evaluation of the contents of microvascular invasion of hepatocellular carcinoma; (3) the gold standard for postoperative pathological results. The final 54 articles are included in this review. Results Currently, there are many methods used to evaluate the microvascular invasion of hepatocellular carcinoma. Common methods include morphological evaluation, texture analysis, diffusion weighted, dynamic enhancement, fat analysis, functional imaging and comprehensive analysis of hepatocytes. Conclusion Various MR methods have different performance in evaluating microvascular invasion. Combined application of multimodal MR images has great potential in comprehensive prediction of microvascular invasion of HCC.
