目的 探讨周围型肺癌中钙依赖细胞粘附分子 (E -Cadherin ,E -cad)的表达及E -cad与肿瘤生长方式及CT表现的关系。方法 应用兔疫组化方法 (SABC法 )对 5 7例周围型肺癌标本进行了E -cad检测 ,并对其中 3 3例标本进行了肿瘤区与瘤肺交界区组织学切片检查 ,以探讨肿瘤的生长方式。全部病例均进行了术前CT扫描 ,对周围型肺癌中E -cad表达及与肿瘤生长方式和相应CT表现的关系进行了探讨。结果 E -cad在周围型肺癌的阳性表达率较正常支气管肺组织显著减少 (Ρ <0 .0 1) ,且随肿瘤分化的降低而相应减少 (Ρ <0 .0 5 ) ,而且肺癌E -cad表达与肿瘤生长方式及CT表现有关 ,呈堆集式生长患者E -cad阳性表达率较呈蟹足式和伏壁式生长者明显为高 (Ρ <0 .0 5 )。在CT表现上 ,当瘤体直径 >3cm、有棘突征、有瘤体邻近胸膜下脂肪线消失或纵隔淋巴结增大者其E -cad表达显著减弱或消失。结论 ①E -cad表达的减弱和消失与肺癌细胞的恶性转化、癌细胞分化、浸润和转移有关。②E -cad的存在与否可能决定着周围型肺癌的生长方式和CT表现
Objective To investigate the expression of E-cadherin (E-cadherin, E-cad) and the relationship between E-cad and tumor growth and CT findings in peripheral lung cancer. Methods Fifty-seven peripheral lung cancer specimens were detected by E-cad with the method of immunohistochemistry (SABC method). Histological sections of tumor area and tumor-lung junction area were examined in 33 of them, The way of growth. All cases were performed preoperative CT scan, the expression of E-cad in peripheral lung cancer and its relationship with tumor growth pattern and corresponding CT findings were discussed. Results The positive expression rate of E-cadherin in peripheral lung cancer was significantly lower than that in normal bronchial lung tissue (P <0.01), and decreased with the decrease of tumor differentiation (P <0. 05) The expression of cadherin was correlated with the tumor growth pattern and CT appearance. The positive expression rate of E-cadherin in stacked growth patients was significantly higher than those in crab-footed and walled-walled growers (P <0.05). In CT findings, E-cad expression was significantly weakened or disappeared when the diameter of the tumor was> 3 cm, there was spinous process, the adjacent subpleural fat line disappeared, or the mediastinal lymph nodes were enlarged. Conclusion ① The decrease and disappearance of E-cad expression are associated with the malignant transformation of lung cancer cells, cancer cell differentiation, invasion and metastasis. ② The presence or absence of E-cad may determine the growth pattern and CT appearance of peripheral lung cancer
