目的分析胸片小阴影总体密集度呈2级或3级改变的Ⅱ期矽肺患者与Ⅲ期患者之间的肺功能损伤程度,为修订尘肺病诊断分期提供参考依据。方法收集我省2002—2007年期间胸部后前位片和肺功能测定资料完整的、确诊Ⅱ期或Ⅲ期的85例矽肺患者,按肺部X射线影像学改变分成3组:观察组(3级密集度),Ⅱ期对照组(2级密集度)和Ⅲ期对照组。结果经统计学分析,3组病例的平均年龄、接尘工龄差异无显著性(P>0.05);工种分布差异无显著性(2=7.565,P>0.05)。经两两比较,观察组的肺通气功能损害程度与Ⅱ期对照组比较,差异有显著性(P<0.05),但与Ⅲ期对照组比较,差异无显著性(P>0.05);Ⅱ期对照组与Ⅲ期对照组比较,差异有显著性(P<0.05)。结论本组调查矽肺病例,胸片呈3级密集度改变的Ⅱ期矽肺的肺功能损害程度与Ⅲ期矽肺相仿,但肺功能损害程度比2级密集度改变的Ⅱ期矽肺严重。
Objective To analyze the degree of pulmonary function impairment between stage Ⅱ silicosis patients with stage Ⅱ and stage Ⅲ patients whose overall density of small shadow in chest X-ray is relatively low, and to provide a reference for revising the stage of pneumoconiosis diagnosis. Methods A total of 85 cases of silicosis patients with complete anterior thoracolumbar spine and pulmonary function tests during 2002-2007 in our province were enrolled and classified into 3 groups according to the pulmonary X-ray findings: the observation group (3 Grade concentration), phase Ⅱ control (grade 2) and phase Ⅲ control group. Results The statistical analysis showed that there was no significant difference in the average age of the three groups (P> 0.05). There was no significant difference between the two groups (2 = 7.565, P> 0.05). There was no significant difference between the observation group and the control group (P <0.05), but there was no significant difference between the two groups (P> 0.05) The difference between the control group and the control group was significant (P <0.05). Conclusion This study investigates the extent of pulmonary dysfunction in stage Ⅱ silicosis with grade 3 density changes in patients with silicosis. The degree of pulmonary function impairment is similar to that in stage Ⅲ silicosis, but stage Ⅱ silicosis with more severe lung function impairment than grade 2 changes.
