目的:探讨和评价微种植体在矫治微笑露龈病例中的临床疗效。方法:随机选择15例于武汉协和医院口腔医学中心进行正畸治疗并伴有露龈笑的患者,在前牙区及后牙区的牙槽骨中植入微种植体,术后即刻加力,每侧前牙垂直向加力100 g,后牙水平向加力200 g。临床观察露龈笑改善后,取出前牙种植钉,继续利用后牙区种植钉关闭剩余间隙。结果:15例患者牙龈暴露量减少(1.59±0.88) mm。头影测量分析显示上中切牙切端至上唇下缘的垂直高度平均减小(2.46±0.90) mm,中切牙临床牙冠长度减少(0.55±0.43) mm、侧切牙临床牙冠长度减少(0.37±0.27) mm、尖牙临床牙冠长度减少(0.50±0.51) mm,且治疗前后差异有统计学意义(n P<0.05)。曲面断层X线片显示治疗后牙根未见明显吸收。n 结论:前牙区微种植体支抗可有效压低上前牙,改善露龈笑,值得推广使用。“,”Objective:To evaluate the clinical efficacy of micro-implants in the patients with gummy smile.Methods:Fifteen patients with gummy smile were randomly selected from dentistry center of Wuhan Xiehe Hospital. Micro-implants were applied in the alveolar bone of the anterior and posterior regions. Orthodontic forces were applied immediately after implant insertion, with 100 g vertical in anterior region and 200 g transverse in posterior region. After clinical observation of relief of gummy smile, anterior implants were taken out, leaving the posterior implants to close the remaining space.Results:The exposure of gingiva in 15 patients was decreased with (1.59±0.88) mm. The cephalometric analysis showed that the distance from central incisal edge to lower border of upper lip was reduced by (2.46±0.90) mm, and the clinical crown length of central incisors, lateral incisors and canines were reduced by (0.55±0.43) mm, (0.37±0.27) mm, and (0.50±0.51) mm, respectively, and the changes had statistical significant (n P<0.05). The panoramic radiographs showed no obvious root absorption after treatment.n Conclusions:The micro-implant anchorage in the anterior teeth can effectively intrude upper anterior teeth and relieve gummy smile.
