Clinical Application of Advanced CNS Tumor MR Imaging including MR Spectroscopy, MR Contrast Perfusion, MR BOLD Functional Imaging and Diffusion Tensor Fiber Tractography have been increasing the role of evaluation of CNS tumor and its mimics as well as monitoring the post treatment status including radiation necrosis and tumor recurrence.MR Spectroscopy provides metabolic imaging for evaluation of Choline, NAA, Cr, lactate and lipid; high Choline, low NAA and Cr and presence of lactate and lipid favoring malignancy.MR contrast perfusion imaging provides the information of regional cerebral blood volume and regional cerebral blood flow; increase of regional cerebral flow and regional cerebral volume favoring malignant tumor and tumor recurrence.Functional MR imaging shows functional cortical localization in relationship to CNS tumor leading the surgical treatment planning.MR Diffusion Tensor Fiber Tractography provides information of white mater fiber displacement vs destruction by CNS tumor which may be helpful for surgical treatment planning.There is a linearly increasing signal to noise ratio and magnetic susceptibility, 3 Tesla is favoring than 1.5 Tesla.
