目的总结基层医院对于新生儿牛奶蛋白过敏(cow’s milk protein allergy CMPA)临床表现的早期识别及诊断、治疗方法。方法回顾性分析23例牛奶蛋白过敏患儿临床表现、实验室检查、治疗方法。结果新生儿牛奶蛋白过敏(CMPA)临床表现不典型,是基层医院不能早期识别的原因之一。主要临床表现为湿疹、呕吐、腹泻、血便及喂养困难等,体重增长多落后于正常新生儿。经牛奶蛋白回避及益生菌治疗症状均得到缓解,体重增长快。结论新生儿牛奶蛋白过敏的临床表现多样,无特异性。早期表现是特应性皮炎(湿疹),逐渐出现胃肠道症状、呼吸道症状等。故应引起基层医院医生重视,早期识别并给予治疗是新生儿正常生长发育的关键。选择深度水解蛋白配方粉治疗牛奶蛋白过敏效果好。
Objective To summarize the early identification, diagnosis and treatment of primary clinical manifestations of cow’s milk protein allergy (CMPA) in primary hospitals. Methods Retrospective analysis of 23 cases of milk protein allergy in children with clinical manifestations, laboratory tests, treatment. Results Neonatal milk proteinuria (CMPA) clinical manifestations of atypical, is one of the reasons that primary hospitals can not be identified early. The main clinical manifestations of eczema, vomiting, diarrhea, bloody stools and feeding difficulties, weight gain more than normal newborns. Avoided by milk protein and probiotics treatment were relieved, rapid weight gain. Conclusion The clinical manifestations of milk protein allergy in newborns are diverse and unspecific. Early manifestation of atopic dermatitis (eczema), the gradual emergence of gastrointestinal symptoms, respiratory symptoms. It should cause grass-roots hospital doctors attention, early identification and treatment is the key to the normal growth and development of newborns.
