目的探讨新式剖宫产术胎头娩出困难的原因及相关处理措施。方法回顾性分析我院2006年1月至2008年7月头位新式剖宫产术1 068例临床资料。结果娩头困难146例,其中主要原因为胎头高浮58例(39.73%),另外,麻醉效果欠佳28例(19.18%),胎头深定22例(15.07%),巨大儿13例(8.90%),切口位置选择不当或切口过小10例(6.85%),产妇肥胖8例(5.48%)等。结论新式剖宫产术中娩头困难是由多种原因造成的,术前要判断麻醉效果、胎头位置、胎儿大小、腹壁肥厚等因素综合考虑,术中出现取头困难时,可采取扩大切口等相应措施顺利娩出胎儿。
Objective To explore the causes of the newborn cesarean delivery and the related treatment measures. Methods The clinical data of 1,068 cases of new cesarean section in our hospital from January 2006 to July 2008 were retrospectively analyzed. Results 146 cases of delivery head difficulties, the main reason for the high fetal head floating in 58 cases (39.73%), in addition, poor anesthesia in 28 cases (19.18%), 22 cases of deep fetal head (15.07%), 13 cases of macrosomia (8.90%), improper incision location or incision in 10 cases (6.85%), maternal obesity in 8 cases (5.48%) and so on. Conclusions The difficulties of delivery in the new cesarean section are caused by a variety of reasons. Factors such as anesthetic effect, fetal head position, fetal size and abdominal wall hypertrophy should be considered preoperatively. Incision and other appropriate measures to deliver the fetus smoothly.
