目的:了解综合性医院病种复杂的中医肝病专科住院患者的医院感染特点。方法:回顾性分析中医肝病专科患者医院感染情况。结果:两年半内中医科住院患者共1 564例,其中肝硬化783例,包括原发性肝癌(PLC)613例,医院感染59例次,例次感染率3.8%,医院感染患者55例,医院感染率3.5%,42例医院感染患者原发病为肝硬化,其中34例并发PLC,PLC院感患者占总医院感染率的65.5%,明显高于其它病种的医院感染率(P=0.000)。感染部位以腹膜感染为主(33.9%),其次为消化系统(25.4%)和呼吸系统(20.3%)(P=0.000)。感染病原包括多种细菌、真菌和病毒,白念珠菌较为常见(35.0%)。影响PLC医院感染患者死亡的主要因素为TNM分期和Child-Pugh分级(P=0.000)。结论:以多病种为特征的综合医院中医肝病专科医院感染患者其原发病以肝硬化和PLC为主,感染部位以腹膜感染为主,白念珠菌为常见病原体;中晚期PLC医院感染患者死亡的因素仍与肿瘤本身情况和肝脏功能有关。
Objective: To understand the characteristics of nosocomial infection in hospitalized patients with complicated traditional Chinese medicine liver diseases. Methods: Retrospective analysis of hospital infections in patients with TCM liver disease. Results: There were 1,564 inpatients in traditional Chinese medicine in two and a half years, including 783 cases of liver cirrhosis, including 613 cases of primary liver cancer (PLC), 59 cases of nosocomial infection, 3.8% of cases of infection, and 55 cases of nosocomial infections. The hospital infection rate was 3.5%. In 42 cases of nosocomial infection, the primary disease was cirrhosis, of which 34 cases were complicated by PLC. PLC inpatients accounted for 65.5% of the total nosocomial infection rate, which was significantly higher than the rate of nosocomial infection in other diseases (P =0.000). The infection site was mainly peritoneal infection (33.9%), followed by the digestive system (25.4%) and the respiratory system (20.3%) (P=0.000). Infectious agents include a variety of bacteria, fungi, and viruses, and Candida albicans is more common (35.0%). The main factors affecting the death of patients with PLC hospital infection were TNM staging and Child-Pugh classification (P=0.000). Conclusion: The main diseases of hospitals with TCM liver diseases in general hospitals characterized by multi-species diseases are mainly cirrhosis and PLC. The infection site is mainly peritoneal infection. Candida albicans is the common pathogen; The factors of death are still related to the condition of the tumor itself and liver function.
