麦芒是我国小麦产区农村常见的结膜囊异物,每年麦收前后常有发生,麦芒进入结膜囊后,随眼睑运动和揉眼常移行到上穹窿结膜隐蔽而不易发现,造成严重的刺激症状,如得不到及时取出与治疗,还会导致严重的并发症,现将近2年来麦芒结膜囊异物50例报道如下: 一、临床资料病例来源于1989年5月至1991年5月的门诊病人,其中男35例,女15例。全部病例均为单眼。年龄最大者75岁,最小者3岁,平均37岁。多数为直接参加麦田劳动者。异物发生后就诊时间最短者为异物入眼后立即来院就诊,时间最长者达56天之久。体征与检查:麦芒进入结膜囊后马上产生强烈的异物感,患眼刺激症状明显,结膜充血,大部分病人合并不同程度的角膜上皮损伤,有的合并化脓性角膜溃疡。检查时先用手拉开上下眼睑,观察球结膜及角膜情况,然后翻转上睑,嘱病人用力向下注视,尽量暴露上穹窿部结膜,异物多能自然露出,必要时可点1％地卡因作表面麻醉。治疗:一旦发现异物应立即取出,用血管钳或镊子把异物挟持牢固,顺势拔出,防止折断残留,因麦芒上长有较多逆心性芒
