目的:探究妇科盆腔手术合并下肢静脉炎的防治及对策。方法:选取2011年12月-2014年12月本院行妇科盆腔手术的114例患者为研究对象。术后发生下肢静脉炎37例为观察组。未发生下肢静脉炎77例为对照组。回顾性分析两组临床资料。结果:观察组的平均年龄、心血管疾病史和恶性疾病史明显高于对照组,差异具有统计学意义(P<0.05)。而在身体质量指数(BMI)、静脉曲张、糖尿病史等方面比较,差异均无统计学意义(P>0.05)。恶性肿瘤患者术后并下肢静脉炎78.6%的概率远高于良性肿瘤17.4%。两组的手术时间比较,差异无统计学意义(P>0.05),但观察组全身麻醉比例78.4%及淋巴清扫的比例35.1%明显高于对照组的全身麻醉比例49.4%及淋巴清扫的比例7.8%,差异具有统计学意义(P<0.05)。结论:对盆腔手术的患者原发病采取积极的护理与治疗,手术操作熟练,缩短手术时间及麻醉时间,术后积极给予抗炎、抗凝、溶栓、营养支持等对症治疗及护理。密切监测患者凝血功能。鼓励患者早日下床活动,穿弹力袜等措施可以有效的预防术后静脉炎的发生。
Objective: To investigate the prevention and treatment of pelvic phlebitis in gynecologic pelvic surgery. METHODS: A total of 114 patients undergoing gynecological pelvic surgery from December 2011 to December 2014 were selected as the study subjects. Postoperative phlebitis occurred in 37 cases of observation group. 77 cases without phlebitis of lower extremity were the control group. Two groups of clinical data were retrospectively analyzed. Results: The average age, the history of cardiovascular diseases and the history of malignant diseases in the observation group were significantly higher than those in the control group (P <0.05). There was no significant difference in body mass index (BMI), varicose veins and diabetes mellitus (P> 0.05). The probability of postoperative 78.6% of the patients with phlebitis of the lower extremities was much higher than that of the benign ones (17.4%). There was no significant difference in operative time between the two groups (P> 0.05). However, the ratio of general anesthesia to lymphadenectomy in 78.4% and 35.1% of the observation group was significantly higher than that in the control group (49.4%) and lymphadenectomy %, The difference was statistically significant (P <0.05). Conclusion: The primary disease in patients with pelvic surgery take active care and treatment, skilled operation, shorten the operation time and anesthesia time, postoperative positive anti-inflammatory, anticoagulant, thrombolysis, nutritional support and other symptomatic treatment and care. Closely monitor the patient’s coagulation function. Encourage patients to get out of bed as soon as possible, wear elastic stockings and other measures can effectively prevent postoperative phlebitis.
