目的探讨核仁组成区蛋白(AgNOR)在冒镜刷检细胞学良、恶性鉴别诊断中的价值.方法49例胃镜刷检细胞学涂片以胶质银技术染色,依据上海方案进行AgNOR观察、以H-E染色涂片及组织学切片相对照.结果随鳞状上皮病变级别增高,AgNOR均数逐渐增高;腺癌细胞AgNOR均数明显高于正常腺上皮细胞(P<0.05);正常细胞与癌细胞AgNOR数目、形态、大小及分布都有差异.应用AgNOR染色观察.使细胞学与组织学的诊断符合率由原来的75.51%提高到83.67%.结论AgNOR在胃镜刷检细胞学中是反映细胞增殖水平较敏感指标,在大规模细胞学普查中可能将有较大的实用价值.
Objective To investigate the value of nucleolar organizer protein (AgNOR) in differential diagnosis of benign and malignant cytology of brushing test. Methods Forty-nine cases of endoscopy brush cytology smears were stained with colloidal silver. According to the Shanghai protocol, AgNOR observation was performed. H-E stained smears and histological sections were compared. The results showed that the average number of AgNORs increased with the grade of squamous epithelium and the number of AgNORs. The average number of AgNORs in adenocarcinoma cells was significantly higher than that in normal glandular epithelial cells (P <0.05). The number, morphology, size and distribution of AgNOR in normal cells and cancer cells Differences. AgNOR staining was used. The diagnostic accuracy of cytology and histology increased from 75.51% to 83.67%. Conclusion AgNOR is a sensitive indicator of cell proliferation in endoscopy brush cytology, which may be of great practical value in large-scale cytological screening.
