目的通过对1例韦格纳肉芽肿伴肺结核患者的药学监护及用药教育案例分析,阐明临床药师和临床医师在临床药物治疗中密切配合的重要性和必要性。方法临床药师在1例韦格纳肉芽肿伴肺结核患者的治疗过程中,与临床医师共同制订治疗方案,监护患者用药全过程并对患者进行用药教育,针对患者病情变化提出合理换药。结果在临床药师参与下,临床医师调整治疗方案,真正做到药物治疗个体化,使患者临床症状得到有效治疗。结论临床药师通过有效的药学监护手段,密切配合临床医师制订科学合理的药物治疗方案,对解决诸如韦格纳肉芽肿伴肺结核这样的疑难、复杂、危重患者的临床治疗问题有重要意义。
Objective To clarify the importance and necessity of close cooperation between clinical pharmacists and clinicians in clinical drug therapy through the case analysis of one case of Wegener’s granulomatosis with pulmonary tuberculosis and the medical education and medication. Methods Clinical pharmacists worked together with clinicians in the treatment of one case of Wegener’s granulomatosis with pulmonary tuberculosis. The clinical pharmacists took care of the patients ’whole course of medication and educated the patients and put forward rational dressing-changing for the patients’ condition. Results With the participation of clinical pharmacists, clinicians adjusted the treatment plan and truly personalized drug treatment, so that the clinical symptoms of patients were effectively treated. Conclusion Clinical pharmacists working closely with clinicians to formulate a scientific and rational drug treatment plan through effective pharmacy supervision means is of great significance to solve the clinical treatment problems of complicated, critically ill patients such as Wegener’s granulomatosis and pulmonary tuberculosis.
