目的观察免疫及锌剂在辅助治疗小儿反复呼吸道感染的效果,为预防和治疗小儿反复呼吸道感染提供依据。方法将2009年1月~2011年1月襄阳第一人民医院RRI患儿156例,随机分为3组,分别为治疗1组、治疗2组和对照组,每组52例,均予抗感染对症支持治疗,另外治疗1组同时予IVIG静脉滴注,治疗2组同时予IVIG静脉滴注和锌硒宝片口服治疗。随访1年,观察疗效,同时对3组治疗前后血清Ig水平进行测试,并进行比较。结果 (1)近期疗效相比,3组间差异性不大,而远期疗效相比,治疗组与对照组之间、两治疗组之间差异有统计学意义。(2)治疗前3组血清Ig水平均低于正常值,3组间比较均差均无统计学意义,两治疗组治疗前后治疗组血清Ig水平其差异均具有统计学意义。结论 RRI患儿存在免疫功能低下,进行免疫或免疫联合锌剂补充辅助治疗对于提高RRI患儿免疫水平,减少及预防感染的发生具有很好的效果,而单纯补充免疫球蛋白在治疗和预防RRI时其效果不如同时补充锌剂。
Objective To observe the effects of immunization and zinc in adjuvant treatment of recurrent respiratory tract infections in children and provide evidence for the prevention and treatment of recurrent respiratory tract infections in children. Methods From January 2009 to January 2011, 156 cases of RRI in Xiangyang First People’s Hospital were randomly divided into three groups: treatment group 1, treatment group 2 and control group, with 52 cases in each group, all of which were given anti-infection Symptomatic supportive treatment, while another group treated with intravenous IVIG intravenously, the treatment group 2 IVIG intravenous infusion and zinc selenium tablets oral treatment. Followed up for 1 year, the curative effect was observed, meanwhile, serum Ig levels of three groups were tested before and after treatment, and compared. Results (1) There was no significant difference between the three groups in the short-term curative effect, but there was significant difference between the two groups in the treatment group and the control group as compared with the long-term curative effect. (2) The serum Ig levels of the three groups before treatment were all lower than the normal values, and there was no significant difference among the three groups. The difference of serum Ig levels between two treatment groups before and after treatment was statistically significant. Conclusion There are immunocompromised children with RRI, immunization or immunization combined with zinc supplementation adjuvant therapy to improve the immune status of children with RRI, reduce and prevent infection has a good effect, and simply immunoglobulin in the treatment and prevention of RRI When the effect is not as good as zinc supplements.
